Program Features:

  • Confidential, onsite health screening
  • Screening includes:
    • Finger-stick for cholesterol and glucose
    • Blood Pressure
    • Pulse
    • Body Mass Index (BMI)
    • Private one-on-one consultation of your results with a nurse
    • Health Risk Assessment Questionnaire (completed prior to your screening)
  • Ongoing Health Coaching in a confidential, private, one-on-one sessions with a licensed nursing professional.
  • O'Brien Fitness Center Membership (Free to City of South Bend Employees)

A: Yes! New for 2014 - expansion of wellness program for spouses and domestic partners.
