All city wellness participants must have their Biometric Screening with OnSite Health Solutions AND the Health Risk Assessment on the portal complete by March 15, 2013. If you do not have these two items complete, you will lose the lower premium insurance for the second quarter. If you have not completed your Biometric Screening contact OnSite at 574-243-5108.
